Team
The
Cahaba
Hospice
care
team
consists
of
an
interdisciplinary
group
that
distinguishes
hospice
care
from
ordinary
home
care.
All
members
are
specially
trained
for
the
care
and
support
terminally
ill
patients
and
their
families
require.
The
patient
and
family
is
the
core
of
Cahaba
Hospice’s
team
and
are
at
the
center
of
all
decision-making.
Meet
our
team:

Medical
Director
and
the
patients.

Personal
Physician
to
direct
and
supervise
care.
- Nurses
experienced
in
pain
management
that
visit
regularly
and
train
caregivers
in
patient
care.
- Home
Health
Aides
to
assist
with
personal
hygiene
and
homemaker
service

Trained
volunteers
to
give
support
and
relieve
caregivers.

Social
Worker
to
give
assistance
and
to
coordinate
services
with
other
community
agencies.
- A
Chaplain
to
offer
help
and
advice
for
emotional
and
spiritual
concerns.

Bereavement
Care
Coordinator
to
organize
follow-up
contacts
for
surviving
family
members.
